7 • Where the Assured Life is a Dependant the original Omang or the original birth certificate of such Dependant. • Where the Assured Life is a Dependant older than twenty-five, proof that such Dependant is or would be permanently dependent on the Policyholder because of mental and/or physical disability; • must provide any other documents required by Kganya Botswana to satisfy the requirements of the Policy including but not been limited to: • a marriage certificate, adoption certificate or other proof sufficient to show that an Assured Life who is adopted or does not have the same surname as the Policyholder was a Dependant or Paid-up Dependant; or • a certified copy of the Botswana Police Services vehicle accident report; or • the Botswana Police Services case docket number.
