l.1.16 1.1.l.7 "Ctaim" means a BE\ Claim or a PA Clairn; "Contribution" means the monthly arnount payable by a Policyholder as a contribution towards ZCC b<penses as detennined in paragraph 2.2; l.1.18 "Commencement Date" means the date fromwhich the amounts relating to Benefits, Premiums and Contributions are to take effect as specified in the Annexure; 1.1.19 1.1.20 1.1.21 1.1.22 1.1.23 "Committee Member" means a Policyholder in Good Standing who has been officially appointed by the ZCC Head office at Moria as a member or shadow member of a committee delegated by the ZCC to perform agreed functions relating to a specific Church registered as such with the ZCC; "Continuation Premium Payment Book" means a numbered and bar� coded Premium Payment Book purchased from a Pay Point by a Policyholder where such Pol!cyholder's Premium Payment Book or prior Continuation Premium Payment Book, as the case may be, no longer has the capacity to affix Receipts within the monthly spaces prescribed; "Date of Accident" means the date of the Accident in which an Insured Life incurred a Permanent Disability; "Date of Death'' means the date of death of an Assured Life; "Date of Payment" means the date a Premiumwas paid by a Policyholder to a Pay Point determined on the basis set out herein; 1.1.24 "Dependant" means in relation to a Policyholder, a person who satisfies the requirements in paragraph 2.8; 1.1.25 "Effective Date'; means the first day of the Month during which the first Policies are issued; 6
