11 DEFINITIONS 1.12 “ CONTRACT ” means the Policyholder Application Form, Payment Book and Annexure. 1.13 “ CONTRIBUTION ” means the monthly amount payable by a Policyholder as a contribution towards ZCC Expenses as specified in the Annexure under the heading PREMIUMS AND CONTRIBUTIONS; 1.14 “ COMMENCEMENT DATE ” means the date from which the amounts relating to Benefits, Premiums and Contributions are to take effect as specified in the Annexure; 1.15 “ COMMITTEE MEMBER ” means a person who has been officially appointed by the ZCC Head office at Moria as a member or shadow member of a Church Branch committee delegated by the ZCC to perform agreed functions relating to a specific Church registered as such with the ZCC; 1.16 “ CONTINUATION PAYMENT BOOK ” means a numbered and bar-coded Payment Book purchased from a Church Branch by a Policyholder where such Policyholder’s Payment Book or prior Continuation Payment Book, as the case may be, no longer has the capacity to affix Receipts within the monthly spaces prescribed; 1.17 “ DATE OF ACCIDENT ” means the date of the Accident in which an Assured Life incurred a Permanent Disability; 1.18 “ DATE OF DEATH ” means the date of death of an Assured Life; 1.19 “ DATE OF PAYMENT ” means the date a Premium was paid by a Policyholder to a Church Branch Committee Member; 1.20 “ KGANYA BOTSWANA ” means Kganya Botswana (registration number 97/817), a company limited by guarantee, registered in accordance with the laws of Botswana, licenced to operate as an Insurance Agent by the Non-Bank Financial Institutions Authority and duly appointed as an Independent Agent by BLIL; 1.21 “ MINIMUM AMOUNT PAYABLE ” means the minimum amount payable for a Personal Accident Benefit as stipulated in the Annexure under the heading PERSONAL ACCIDENT BENEFITS; 1.22 “ MAXIMUM AMOUNT PAYABLE ” means the maximum amount payable for a Personal Accident Benefit as stipulated in the Annexure under the heading PERSONAL ACCIDENT BENEFITS;
