12 1.23 “ MONTH ” means a calendar month; 1.24 “ PAYMENT BOOK ” means a numbered and bar-coded book purchased by a Policyholder from a Church Branch Committee Member and includes a Continuation Payment Book and Replacement Payment Book; 1.25 “ PERMANENT DISABILITY ” means a permanent disability as specified in the Annexure under the heading PERSONAL ACCIDENT BENEFITS and which is incurred at the time of and as a direct result of an Accident which will in all probability continue for the remainder of the Assured Life’s life; 1.26 “ PERCENTAGE BASE AMOUNT ” means the percentage of Base Amount stipulated in the Annexure under the heading PERSONAL ACCIDENT BENEFITS; 1.27 “ PERSONAL ACCIDENT BENEFIT ” means the amount payable by BLIL as set out in the Annexure under the heading PERSONAL ACCIDENT BENEFITS; 1.28 “ POLICY ” means an insurance policy between BLIL and the Policyholder; 1.29 “ POLICYHOLDER ” means a member of ZCC Botswana who satisfies all Policyholder eligibility requirements as outlined in the Policy; 1.30 “ PREMIUM ” means the amount which is payable each Month by: • a Policyholder as consideration for the Burial Benefit and Personal Accident Benefit; • ZCC Botswana as consideration for the Thari ya Baruti Benefit. 1.31 “ POLICYHOLDER APPLICATION FORM ” means a Policyholder Application Form contained in a Payment Book; 1.32 “ RECEIPT ” means a numbered coupon of Kganya Botswana which is inserted in a Payment Book by a Church Branch Committee Member against payment of a Premium and Contribution for the amount specified on such coupon; 1.33 “ REPLACEMENT PAYMENT BOOK ” means a numbered and bar-coded Payment Book purchased from a Church Branch Committee Member by a Policyholder where that person’s Payment Book or Continuation Payment Book, as the case may be, has been misplaced; 1.34 “ SERVICES ” means Road Side Assistance Services provided in terms of the Policy; DEFINITIONS
