14 2. POLICYHOLDERS 2.1 Eligibility Subject to the other requirements contained herein, a Policyholder shall be a person who: • is a member of ZCC Botswana; • is 18 (eighteen) years or older; • has purchased a Payment Book; and • has paid an initial Premium and Contribution and will continue to be a Policyholder for as long as such person remains a member of ZCC Botswana. A person who satisfies the requirements above, will be a Policyholder from the later of: • the date of issue of the Payment Book; or • the Date of Payment of an initial Premium and Contribution. 2.2 Policyholder Amendments An Amended Policyholder Application Form must be completed with a Church Branch Committee Member where a Policyholder wishes to: • amend or update the Policyholder’s personal particulars shown in the Payment Book; and/or • change the Beneficiary. The onus shall be on the Policyholder to ensure that: • a copy of the Amended Policyholder Application Form is inserted in the Payment Book; and • the pre-printed sequence number of the Amended Policyholder Application Form and the date of change are recorded in the Payment Book. Unless otherwise determined by Kganya Botswana, any amendment made will not be effective until the original Amended Policyholder Application Form signed by the Policyholder has been returned to and received by Kganya Botswana. 2.3 Beneficiary Nomination A Beneficiary must not be a minor or a person otherwise legally disqualified from receiving the Burial Benefit or Thari ya Baruti Benefit.
