1 DECLARATION In applying to become or continue as a Policyholder I confirm that: 1. I am a member of Zion Christian Church in Botswana and I am abiding by Church Regulations. 2. I undertake to read the Policy wording contained within this Payment Book and associated Annexure and agree to abide by such terms and conditions as amended from time to time. 3. I understand that this Policy wording is a summary of the Master Policy and Individual Policy terms and conditions as agreed upon between the Zion Christian Church in Botswana and Botswana Life Insurance Limited (the Insurer). I am aware that should I require it, a copy of the Master Policy of Insurance with the Insurer may be obtained upon request, free of charge, from Kganya Botswana (the Insurance Agent). 4. I am aware that I am only entitled to Benefits and Services if I pay my Premiums and Contributions monthly in accordance with applicable Good Standing rules and I remain a member of Zion Christian Church in Botswana. 5. I am aware that Premiums and Contributions are due on the first day of each calendar month and must be paid to an authorised Church Branch Committee Member on or before the last day of such month. Late payment of a Premium and Contribution will not be condoned regardless of the reasons therefore. 6. I am aware that I should never pay a Premium and Contribution unless an official receipt showing the date and the amount paid is inserted in this Payment Book. I am not required to and must never pay more than the amount shown on the receipt. 7. I am aware that I can forfeit Benefits and Services at any time by making no further Premium and Contribution payments. It is important to remember that such non-payment normally leads to loss of valuable Benefits and Services and should be avoided where possible. 8. I am aware that if I decide to no longer pay my Premiums and Contributions I will not be refunded Premiums and Contributions already paid by me, subject to Cooling Off rights afforded. 9. I understand that all Benefits and Services are offered as a package and that individual Benefits and Services are not available on a selective basis. 10. I have not been pressured into this Policy application and do so out of my own free will. 11. I have not been provided with any financial advice from any Zion Christian Church Committee Member. If I need financial advice I must obtain the services of an accredited financial advisor. 12. I alone have determined my own ability to afford the prevailing monthly Premium and Contribution.
