34 1. THE POLICY • The Policy is a life policy as defined in the Insurance Industry Act. It is a contract of insurance made by an insurer classified under Section 37(3) of such Act. • The Annexure which forms part of this Payment Book details: • nature and extent of the Benefits and Services available to you, as Policyholder; • breakdown of the Premium payable; • breakdown of the total Monthly amount payable (including the Contribution to ZCC Botswana) • Benefit levels; • Base Amount; • Minimum Amount Payable; • Maximum amounts Payable; • Percentage Base Amount; and • Procedure to submit a Claim. • The Premium and Contribution payable in terms of Pukwana ya Kganya are to be paid to your local Church Branch Committee Member who has been mandated to perform various administrative functions in terms of a mandate provided to them by Kganya Botswana. • In return for the Premium and Contribution that you pay every month to your Church Branch Committee Member you are to be issued with an official receipt – facsimile reproduced below – to be inserted into this Payment Book. Never pay an amount unless such a receipt is issued. Also, you are not required to and must never pay more than the amount shown on the receipt. You must also ensure that not more than two monthly Premiums and Contributions are paid in advance. • The monthly Premium and Contribution will vary in line with the requirements of Zion Christian Church as well as the cost of providing Benefits, Services and administration. Every effort is made to only increase the Premium and Contribution every 2 (two) years with effect from 1 April. Notification of any increase will be given to you by Kganya Botswana through your Church Branch Committee Members. • The cover provided is on a month by month basis. There are no restrictions, penalties or other related effects upon withdrawal. • Your Policy does not have an investment or surrender value. IMPORTANT INFORMATION
