35 • Upon taking out the Policy you are required to pay for the Payment Book. To ensure that cover is maintained you must pay your monthly Premium and Contribution. There are no other levies or charges payable. • Any questions you may have regarding Pukwana ya Kganya or any of the Benefits or Services provided in terms thereof, should be addressed to any one of the Kganya Service Centres (see Annexure: Contacts) or the Kganya Botswana Office: Kganya Botswana (KB): Unit 19 Plot 145 Lake View Office Park Gaborone International Finance Park Gaborone Tel: 267 319 0139 Email: complaints@kganya.co.bw • In the event that a Claim for any Benefit provided under the Policy is declined, BLIL will provide the beneficiary with a written notice advising the reason for the decision within 10 (ten) days of taking the decision. Such beneficiary may within a period of not less than 90 (ninety) days of the relevant decision make representations to BLIL in respect of such decision. 2. THE ADMINISTRATOR • Kganya Botswana (KB), registration number 97/817, is a company Limited by Guarantee. • KB has been issued with a license number 2/10/648 by the Non- Bank Financial Institutions Regulatory Authority (NBFIRA) in terms of the Insurance Industry Act, to act as an Insurance Agent on behalf of Botswana Life Insurance Limited (BLIL). In terms hereof, KB has been authorised to render intermediary financial services with regard to Long Term Insurance, as specified in the Insurance Industry Act. • KB has also been appointed by BLIL to act as its Insurance Agent and to render certain administration services on behalf of BLIL in respect of insurance policies issued as part of Pukwana ya Kganya. • KB holds the requisite professional indemnity cover.
